I/We beg to apply for permission to make or extend excavation on the site described for the purpose of
2. I/We attach the site plan in quadruplicate (three blue prints and one regular plan on oil cloth duly signed by me (as) showing.
(i) the position of the site to be excavated in relation to its surroundings.
(ii) the depth of quarrying to be done and
(iii) the sanctioned layout.
3. I/We undertake to abide by all the Directions of the Authority issued under provision of Section 6 of the Act.
4. I am/We are the owner/owners, lessee/lessees of the land for which I am/we are applying.
5. I/We remit a sum of Rs. /- being application fee.

Form D – Layout for means of access to road.

I/We beg to apply for permission to layout means of access to the road described below:
2. I/We attach the site plan in quadruplicate (three blue prints and one regular plan on oil cloth duly signed by a Licence Architect) showing (i) the position of the site for laying out means of access to the road in relation to its surrounding, (ii) the sanctioned layout of the surrounding area, (iii) the gradient of the proposed means of access and (iv) details of road such as land width, cross-sections, type of crust etc.
3. I/We undertake to abide by all the Directions of the Authority issued under provisions of Section 6 of the Act.
4. I am/We are the owner/owners lessee/lessees of the land for which I am/We are applying.
5. I/We remit a sum of Rs. 50/- being application fee.
